Dentists very often face challenging cases which present severe bone atrophy. Bone loss in the result of the extractions of teeth, after 3 months is has been reported a medium bone loss at the level of the socket of more than 3 mm. It is even more critical after many years of an edentulous gap being present where the alveolar crest can become knife edge like or even leave only the basal bone.

Now a days patients are requesting more oftenly a fixed prosthetical solution to restore the missing teeth and want to undergo as few surgical procedures  as possible. In other words dentistry is following the trend of less invasive procedures but with high degree of predictability.

To rehabilitate severely atrophic patients with a fixed prosthetical solution the typical approach is to regenerate and build up the bone and then, after a long healing period, place the implant fixtures.  

The mid term results however are not ideal. The amount of bone regeneration is not always predictable and it might even undergo resorption in time. This approach shows high morbidity for the patient and very often is associated with multiple surgical procedures.

From the past  comes an alternative implant solution, namely  subperiosteal implants. The success rate in the past was quite poor due to the rudimental technique utilized for their production. They were designed on the cast of the patient’s bone taken with impressions and produced by using the lost wax technique. Their lack of precision with misfitting and impurities was a crucial problem that quite often lead to a rapid failure. The other major problem was that they were often designed by Lab technicians and not by Maxillo Facial Surgeons and Bio-Engineers. For these reasons they were abandoned.

Now, CAD CAM has made it possible to improve on this older implant concept and turn it into a completely whole new solution. The project design of the sub-periosteal implants is now made on a 3D virtual model, prosthetically oriented and printed in Titanium with selective laser melting and is  excellent in being able to distribute the masticatory forces physiologically. 

Eagle grid at present has already a 7 years follow up with hundreds of cases treated with success. This new technology has made it possible to overcome previous anatomical and physiological limits.
